- Operations: We may use your data for staff training, quality assessment, and appointment reminders. Business Associates will protect your data similarly.
- As Required by Law: We may disclose information under federal, state, or local law.
- Military and National Security: Disclosures may be made to military authorities or for national security purposes.
- Research: Permitted under strict privacy regulations, possibly using de-identified data.
- FDA: Medical information may be disclosed for product safety and recall purposes.
- Abuse, Neglect, Domestic Violence: Disclosures permitted if you are a suspected victim, per legal requirements.

Uses and Disclosures Requiring Authorization
Disclosures for marketing, psychotherapy notes, or sales of health info require written consent. Authorizations may be revoked.

- Right to Confidential Communications: You may request alternate forms or locations of communication.

- Right to Accounting of Disclosures: You may request a history of disclosures up to six years prior.
- Right to Paper Copy: You may request a printed copy of this Notice anytime.
Breach Notification
You have the right to be notified of any breach involving your unsecured health information.
